2022-2023 Sublette County Community Needs Assessment

High Country Behavioral Health is responsible for overseeing Community Services Block Grant (CSBG) funds that assist in reducing poverty, improving living conditions and increasing economic self-sufficiency in Sublette County. As a pre-requisite to receiving these funds, Sublette County must complete a Community Needs Assessment every three years. High Country Behavioral Health will use this needs assessment to prepare their three-year Community Action Plan.
- The extent of poverty in Sublette County that includes data specific to poverty and its prevalence related to gender, age and race/ethnicity for Sublette County; - The cause and conditions of poverty and factors that intensify poverty; - Public and private human service agencies and resources that are available to offset the creation or impacts of poverty; - Gaps in services to the low income; - Priorities for meeting the needs of the low income; - Barriers in providing services to the low income; and
Four strategies are used in obtaining the information:
- Surveys distributed to low-income clients of community agencies that serve the low income; - Surveys distributed to over 100 community agencies in Pinedale, Marbleton and Big Piney; - Dialogue on Poverty community meetings that will be held throughout Sublette County and include the following:

The purpose of these meetings is to determine individual needs of the low income, perception of poverty in the community, gaps in services and potential solutions to meet those needs and gaps. - Local, state and regional data resources are researched for key demographic information as related to poverty and the economic conditions in Sublette County. - Final results of the needs assessment will be discussed with High Country Behavioral Health and the Tripartite Board.
High Country Behavioral Health and the Tripartite Board is asking the community to assist with this project by completing the surveys and attending the Dialogue on Poverty community meetings.
